What is zepatier?
- The drug Zepatier contains a combination of elbasvir and grazoprevir. Elbasvir and grazoprevir are hepatitis C (HCV) antiviral drugs that proliferate in your body.
- Drug is used to treat chronic hepatitis C genotype 1 or 4 in adults. This medicine is sometimes taken with a medicine called ribavirin (Copegus, Rebetol, Moderiba, Ribasphere).
- Drug should only be used if your HCV has a specific genetic marker, which your doctor will check.
The mechanism of action of Elbasvir and Grazoprevir
- The hepatitis C virus is a disease that can damage the liver. A virus is a very small pathogen that makes copies of itself to survive by a process called replication. The hepatitis C virus has at least seven different strains, also known as genotypes.
- Drug is a direct acting antiviral drug (DAA). DAA is a group of drugs that directly blocks the ability of the hepatitis C virus to make copies of itself.
- Drug is a combination of two DAAs. One is grazoprevir, a protease inhibitor, and the other is elbasvir, which is an NS5A inhibitor. Grazoprevir interferes with the production of fragments needed to build new viral particles, stopping the production of new hepatitis C virus. Elbasvir works by interfering with a protein needed to complete the life cycle of the hepatitis C virus in liver cells.
- Treatment with DAA can usually cure a person from hepatitis C infection and may reduce the likelihood of further health problems, such as liver failure and liver cancer. However, a person may become infected again. For more information about preventing hepatitis from being passed from one person to another, see Prevention and Harm Reduction.

How long will treatment with zepatier 5mg / 100ml be used?
How long is zepatier treated?
In most cases, the duration of treatment with Drug is based on pre-existing liver damage and, to some extent, based on a person’s genotype and past treatment experience.
The following group will take Zepatier for eight weeks without ribavirin:
Genotype 1b has never been treated (untreated) without serious liver damage (cirrhosis)
The following groups will use Zepatier for 12 weeks without ribavirin:
- Genotypes 1 or 4 people have never been treated
- Genotype 1 or 4 previously used peg-interferon + ribavirin but relapsed (relapse is when the virus is not detectable during treatment but returns after treatment)
- Genotype 1 previously used a protease inhibitor, such as telaprevir (Incivek), boceprevir (Victrelis) or simeprevir (Galexos), but relapsed.
- The 1b genotype previously used peg-interferon + ribavirin: a protease inhibitor but did not complete treatment because of high levels of hepatitis C virus
The following groups will treat for 16 weeks with ribavirin:
- genotype 1a previously used peg-interferon + ribavirin: a protease inhibitor but discontinued treatment because of prolonged high levels of hepatitis C virus
- genotype 4 previously used peg-interferon + ribavirin but discontinued treatment because of prolonged high levels of hepatitis C virus
Genotype 3
People with untreated genotype 3 virus will take Zepatier with sofosbuvir for 12 weeks. Drug is not approved in Canada for people infected with genotype 3 previously treated but not cured.
Stick to treatment
All medicines work best when they are taken exactly as prescribed and instructed. This means taking the medication at the right dose and at the right time throughout your entire treatment period. Skipping doses or stopping treatment altogether means the treatment may be ineffective and the chance of being cured is lower.
With some medications for hepatitis C, it is possible for the virus to become resistant if not used as directed. In this case, the drug will not be effective against the virus and cannot be used to treat infections.
Instructions for use of zepatier drugs
Use Zepatier exactly as prescribed by your doctor. Do not use this medicine in larger or smaller amounts or for longer than recommended.
You can use drug before or after meals.
Take the medicine at the same time every day.
Hepatitis C is usually treated with a combination of other drugs. Use all medications as directed by your doctor. Every person with chronic hepatitis C should remain under the care of a doctor.
You should not stop using this medicine suddenly. Stopping suddenly can make your condition worse.
If you have ever had hepatitis B: Zepatier may cause this condition to come back or get worse. You will need liver function tests during treatment and for several months after you stop using this medicine.
Store drug at room temperature away from moisture and heat. Keep each pill in the blister pack until you use it.

Pregnancy: Zepatier is sometimes used in combination with ribavirin. Ribavirin can cause birth defects or death to an unborn baby. Tell your doctor if you are pregnant or plan to become pregnant. You may need to have a negative pregnancy test before using these drugs together, and every month during your treatment.
- If you are a woman, do not use drug with ribavirin if you are pregnant.
- If you are a man, do not use this combination if your partner is pregnant. The fetus may also be harmed if a man fathers the child while he is taking ribavirin.

On January 28, 2016, the FDA approved Zepatier (elbasvir and grazoprevir), in combination with ribavirin (RBV) or not, for the treatment of chronic hepatitis C virus infection (HCV) GT1 or GT4 in adults.
Hepatitis C can lead to impaired liver function. Most people infected with HCV have no symptoms until they show signs of liver damage, which usually lasts a few years. People with chronic HCV infection are at risk of developing cirrhosis after many years and leading to dangerous complications such as bleeding, jaundice or eyes, ascites, infection or liver cancer. According to the Centers for Disease Control and Prevention, there are about 3 million Americans infected with HCV, of which the most common is HCV GT1, while GT4 is one of the less common types. . According to Edward Cox,
Zepatier is a 1-day combination daily fixed dose of an NS5A inhibitor (elbasvir 50mg) and a protease inhibitor NS3 / 4A (grazoprevir 100mg). Drug has been recognized by the FDA as a “groundbreaking therapeutic product” for the treatment of chronic HCV GT1 infection in patients with end-stage renal disease on hemodialysis and for the treatment of chronic HCV GT4 infection. Zepatier has a high cure rate in many patients with chronic HCV infection, including patients with mild cirrhosis (compensated cirrhosis), patients with all-stage renal failure and patients with bilateral infection. HIV-1 and HCV.
