What is Palbonix and what is it used for?
Palbonix medicine is an anti-cancer medicine that contains the active ingredient palbociclib.
It is also used to treat patients with certain types of breast cancer (hormone receptor positive, human epidermal growth factor receptor negative 2) that has spread beyond the original tumor or to other agencies.
It is given along with aromatase inhibitors or fulvestrants, used as hormonal anticancer therapy.
How does Palbonix work?
- Breast cancer occurs when cells in the breast begin to grow and divide uncontrollably. If your breast cancer is found to be hormone receptor positive (HR+), this means that hormones in your body are involved in making cancer cells grow and multiply.
- Estrogen is a hormone that affects certain processes inside cancer cells. It makes two enzymes (proteins that help chemical reactions happen) become overactive. These enzymes are called cyclin-dependent kinases 4 and 6 (CDK 4 and 6).
- CDKs 4 and 6 are involved in regulating how all cells grow and multiply. When they are overactive in cancer cells, they cause cancer cells to grow and divide faster.
What you need to know before taking Palbonix medicine
Palbonix should not be used
- If you are allergic to palbociclib or any other ingredients of this medicine.
- The use of preparations containing st. John’s wort while you are taking it.
Warnings and Precautions
Talk to your doctor, pharmacist or nurse before taking your medicine.
Palbonix can lower your white blood cell count and weaken your immune system. As a result, you may be at greater risk of infection while you are taking it.
Tell your doctor, pharmacist or nurse if you experience signs or symptoms of an infection, such as chills or fever.
You will have regular blood tests during treatment to check if your blood cells (white blood cells, red blood cells, and platelets) are affected.
Palbonix can cause severe or life-threatening pneumonia during treatment. Leads to death. Tell your healthcare provider right away if you have any new or worsening symptoms including:
- Shortness of breath or shortness of breath
- Ho khan
- Chest pain
Children and adolescents
- Palbonix is not for use in children or adolescents (under 18 years of age).
Dosage and how to use Palbonix
- Palbonix dosage is a 125 mg capsule taken orally once daily for 21 consecutive days after a 7 day treatment break to cover a complete cycle of 28 days. Should be taken at the same time as food.
- When co-administered, the recommended dose of Letrozole is 2.5 mg orally once a day continuously throughout a 28-day cycle. Please refer to Letrozole for complete prescribing information.
- When taken together with the drug, the recommended dose is 500 mg on days 1, 15, 29 and once a month thereafter. Please refer to Fulvestrant for complete prescribing information.
- Patients should be encouraged to take their Palbociclib dose at approximately the same time each day
- Palbonix tablets should be swallowed whole (do not chew, crush or open them before swallowing). Capsules should not be eaten if they are broken, cracked, or otherwise not intact.
- Premenopausal women treated with palbociclib plus optimal therapy should be treated with a luteinizing hormone-releasing hormone (LHRH) agonist according to current standards of clinical practice.
What happens if I miss a dose?
- If you forget to take a dose, skip that dose and take your next dose at your usual time. Do not take an extra dose to make up for the missed dose.
- To make sure you don’t miss a dose, try setting a reminder in your phone. A medication timer can also be helpful.
Do I need to use this Palbonix long term?
Palbonix is used as a long-term treatment. If you and your doctor determine that the medicine is safe and effective for you, you will likely take it long-term.
Possible side effects of Palbonix
Like all medicines, this one can cause side effects, although not everyone gets them.
Contact your doctor immediately if you have any of the following symptoms:
- Fever, chills, weakness, trouble breathing, bleeding or easy bruising can be signs of a serious blood disorder.
- Shortness of breath, dry cough, or chest pain could be signs of pneumonia.
Common side effects after using Palbonix
- Low blood count. Your white and red blood cells and platelets may temporarily decrease. This can increase the risk of infection, anemia, or bleeding.
- Tired
- Acute respiratory failure
These are less common side effects for patients:
- Nausea
- Mouth Sores
- Hair loss / thinning
- Diarrhea
- Reduced appetite
- Vomiting
- Peripheral neuropathy (numbness/tingling of fingers/toes)
- Weak
- Epistaxis
A rare but serious side effect of Palbonix is a blood clot in the lungs (pulmonary embolism (PE)). You should seek emergency help and notify your healthcare provider right away if you have sudden chest pain and trouble breathing.
Notify your healthcare provider within 24 hours if you notice swelling, redness, pain, or warm skin in your leg or arm (signs and symptoms may include blood clot in the arm or leg.)
Not all side effects are listed above. Very rare side effects – occurring in about 10% of patients – are not listed here. But you should always notify your healthcare provider if you experience any unusual symptoms.
When to contact your doctor or healthcare provider
Contact your healthcare provider right away, day or night, if you experience any of the following symptoms:
- Fever 100.4º F (38º C) or higher, chills (possibly signs of infection)
- Sudden chest pain and difficulty breathing
The following symptoms require medical attention, but are not an emergency:
- Nausea (affects ability to eat and is inconsistent with prescribed medication)
- Vomiting (vomiting more than 4 times in a 24-hour period)
- Diarrhea (4 episodes in 24 hours)
- Unusual bleeding or bruising
- Black or black stools or blood in the stools
- There is blood in the urine
- Extreme fatigue (inability to perform self-care activities)
- Mouth ulcers (redness, swelling, or ulcers)
- The leg or arm is swollen, red, painful, or warm to the touch.
Always notify your healthcare provider if you experience any unusual symptoms.
Palbonix drug interactions
- CYP3A Inhibitors: Avoid concomitant use of Palbonix with strong CYP3A inhibitors. If a strong inhibitor cannot be avoided, reduce the dose of Palbociclib.
- CYP3A Inducers: Avoid concomitant use of Palbonix v2i with strong CYP3A inducers.
- CYP3A Substrates: The dose of sensitive CYP3A4 substrates with narrow therapeutic indices may be reduced when co-administered with Palbociclib.
- Other drugs may affect palbociclib, including prescription and over-the-counter medicines, vitamins, and herbal products. Tell your doctor about all your current medicines and any you start or stop using.
Precautions Palbonix 125mg
Pregnant women
- There are no adequate studies in this subject.
- Palbonix medicine can harm an unborn baby. If the drug is used in women of childbearing potential, advise the patient to avoid pregnancy. If the patient becomes pregnant while taking this medicine, the patient should be informed of the potential risk to the fetus.
- If women of childbearing potential are taking Palbonix, patients should use adequate contraception during therapy and for at least 21 days after completion of therapy.
Breastfeeding Women
It is not known whether palbociclib is excreted in human milk. Use caution when using this object.
Elderly people over 65 years old
Population pharmacokinetic analysis was performed on data from 183 patients with cancer between the ages of 22 and 89 years. There were no clinically important differences in Palbociclib exposure in patients ≥65 years of age compared with patients.
Driving and using machines
Fatigue is a very common side effect. If you feel unusually tired, be especially careful when driving or using machinery.
