What is Herceptin?
Herceptin is an intravenous drug that is part of a chemotherapy regimen used to prevent recurrent breast cancer and treat metastatic breast cancer.
It belongs to a class of drugs called monoclonal antibodies. Other monoclonal antibodies include rituximab (Rituxan) and gemtuzumab ozogamicin (Mylotarg).
More information about Herceptin
The doctor will administer Herceptin dosing, in the form of a series of intravenous or subcutaneous infusions, administered subcutaneously, over a period of 1 week 3.
Currently, people with early breast cancer undergo a 1-year Herceptin course.
- Studies have shown that a 1-year treatment is more beneficial than a 6-month treatment. Extending Herceptin therapy after 1 year does not seem to have any benefits.
- Once the cancer has spread to other organs or tissues, your doctor may recommend using pertuzumab, Herceptin and paclitaxel together.
- In treating this type of cancer, Herceptin is only useful until it no longer shows a controlled effect on cell reproduction.
Indications for treatment of Herceptin drug
Herceptin may be used to treat:
- HER2 breast cancer early positive, after surgery or radiation and chemotherapy, to reduce the risk of cancer coming back
- HER2-positive breast cancer progresses from metastatic breast cancer, to slow cancer growth and increase survival
- Positive HER2-positive gastric cancer spreads out of the stomach
- HER2 positive esophageal cancer progresses, affecting the area where the esophagus meets the stomach
If you have breast, esophageal or stomach cancer, tests will be done to check if your cancer is HER2 positive before Herceptin is offered.

The mechanism of action of Trastuzumab
Trastuzumab may help control the growth of cancer cells containing high levels of epidermal growth factor receptor 2 (HER2):
- HER2 is found in all human cells. It controls cell growth and repair.
- But high levels of HER2 are found in some types of breast, esophageal, and stomach cancers, helping cancer cells grow and survive.
- These are called HER2 positive cancers. About one fifth of breast and stomach cancers are HER2 positive.
Trastuzumab works by blocking the action of HER2 and encouraging the immune system to attack and kill cancer cells.
Dosage of Herceptin medication
Weekly transmission schedule:
- initial loading dose 4 mg / kg / 90 minutes; next 2 mg / kg / week
- infusion over 30 minutes if the previous dose is well tolerated.
Schedule of infusion every 3 weeks:
- initial loading dose 8 mg / kg, 3 weeks after 6 mg / kg & then repeat dose 6 mg / kg every 3 weeks
- infusion over a period of about 90 minutes for the first infusion and for about 30 minutes for subsequent infusions if the previous dose is well tolerated.
Forgot dose:
- ≤ 1 week: use the usual dose (2 mg / kg weekly regimen, 6 mg / kg 3-week regimen) as soon as possible.
- Do not wait until the next dose as scheduled.
The next maintenance dose will follow the previous infusion schedule. > 1 week: repeat dose loading of 4 mg / kg weekly regimen, 8 mg / kg 3-week regimen for 90 minutes,
The next maintenance dose: 2 mg / kg weekly regimen, 6 mg / kg 3 week regimen will be calculated from that time.
Elderly: no dose reduction is necessary.
Precautions before using Herceptin
You should not be treated with Herceptin if you are allergic to trastuzumab.
To make sure Herceptin is safe for you, tell your doctor if you have ever had:
- heart disease;
- congestive heart failure;
- heart attack;
- Any allergies or breathing problems.
Trastuzumab can cause heart failure, especially if you have heart disease
If you are taking some other cancer medications:
- daunorubicin,
- doxorubicin,
- epirubicin
- idarubicin
Tell your doctor about all medications you are taking including prescription and over-the-counter medicines.
Do not use Herceptin if you are pregnant: This medication may harm or kill the unborn baby. Use effective birth control to prevent pregnancy while you are using this medication and for at least 7 months after the last dose.
If you become pregnant while using this medicine or within 7 months after you stop, tell your doctor right away.
Herceptin can pass into sesame milk and harm a baby. Do not use when Herceptin breast-feeding while using this medication and until 7 months after your last dose. Ask your doctor about any risks if you are breastfeeding.
Side effects of Herceptin
Get emergency medical help if you have signs of an allergic reaction to Herceptin: hives; shortness of breath; swelling of your face, lips, tongue or throat.
Call your doctor right away if you have the following:
- cough, fever, wheezing or difficulty breathing;
- fast heartbeat;
- chest pain spreads to your jaw;
- swelling in the lower leg, rapid weight gain;
- blisters or ulcers in the mouth, red or swollen gums, trouble swallowing;
- low blood cell count: fever, chills, fatigue, skin sores, easy bruising, unusual bleeding, pale skin, cold hands and feet,
- signs of tumor cell disruption: confusion, weakness, muscle cramps, nausea, vomiting, fast or slow heart rate, decreased urination, tingling in the arms and legs or around your mouth.
Common Herceptin side effects may include:
- nausea, diarrhea, weight loss;
- headache;
- Trouble sleeping, feeling tired;
- rash;
- Mouth Sores;
- fever, chills, cough or other signs of infection;
- altered sense of taste;
- Cold symptoms such as nasal congestion, sinus pain, sore throat.
