What is Darzalex? Indications for treatment of Daratumumab?
Darzalex (daratumumab) is a prescription drug used to treat a type of blood cancer called multiple myeloma. Darzalex is not chemotherapy, it is a monoclonal antibody targeted to help slow or stop the progression of multiple myeloma in many ways.
Of the four multiple myeloma (MM) therapies approved by the US Food and Drug Administration (FDA) in 2015, only Darzalex has single-agent activity and is approved based on superiority. its with regard to the existing treatments.
Darzalex is a CD38 cytologic antibody indicated for the treatment of patients with multiple myeloma:
- Combination with lenalidomide and dexamethasone drugs in newly diagnosed multiple myeloma patients who are not eligible for autologous stem cell transplantation and in those who have received at least one previous drug to treat multiple myeloma.
- In combination with drugs bortezomib, melphalan and prednison in newly diagnosed multiple myeloma patients are not eligible for autologous stem cell transplant.
- In combination with drugs bortezomib, thalidomide and dexamethasone in newly diagnosed multiple myeloma patients are eligible for autologous stem cell transplantation.
- Combined with bortezomib and dexamethasone drugs in people who have received at least one previous drug to treat multiple myeloma.
- Combination with pomalidomide and dexamethasone in people who have received at least two previous drugs for the treatment of multiple myeloma, including lenalidomide and a proteasome inhibitor (PI).
- Alone in individuals who have received at least three previous drugs, including proteasome inhibitors (PIs) and immunomodulatory or non-responsive PI and immunomodulatory drugs.
The mechanism of action of Daratumumab
Many tumor cells may not be identified by your body, allowing them to grow. Daratumumab attaches itself to the CD38 protein on the surface of many tumor cells, as well as on some other types of cells, such as red blood cells.
Daratumumab directly destroys many tumor cells and allows your immune system to identify and destroy them. Due to the way Daratumumab works, it can also affect normal cells.
Contraindicated
Darzalex is contraindicated in patients with a history of severe hypersensitivity or an allergy to daratumumab or any component of the formulation.
Do not use Darzalex for children or adolescents under 18 years of age

Dosage and usage of Darzalex
Dosage recommended
Your doctor will calculate the dose and schedule of Darzalex. The dose of Darzalex will depend on your body weight.
- The usual starting dose of Darzalex is 16 mg per kg of body weight. Darzalex may be given alone or together with other drugs used to treat multiple myeloma.

Darzalex will be given to you by a doctor or nurse in the hospital or clinic by giving an IV (intravenous) into your vein.
The doctor or nurse in your hospital or clinic will decide the time between doses as well as the number of treatments you will receive.
Your doctor or nurse in the hospital or clinic will give you the medicine before each dose of Darzalex and after each dose of Darzalex to help reduce the risk of an infusion reaction.
When offered alone, Darzalex is given as follows:
- Once a week for the first 8 weeks
- Then every 2 weeks for 16 weeks
- Then every 4 weeks as long as your condition does not get worse
When Darzalex is taken along with other medicines, your doctor can change the time between the dose and how many treatments you will receive.
Warnings and precautions when using Darzalex
Hepatitis B: Patients who have had or may have been infected with hepatitis B because Darzalex can make the hepatitis B virus active again.
Anemia: Darzalex may cause low red blood cell levels. If you experience symptoms of decreased red blood cell counts such as difficulty breathing, feeling unusually tired or pale skin, contact your doctor as soon as possible.
Bleeding: Darzalex can reduce the number of platelets in the blood, which can make it difficult to stop bleeding cuts.
Infection: As well as destroying cancer cells, Darzalex can reduce the number of white blood cells that fight infections in the body.
Infusion reaction: This drug may cause hypersensitivity reactions or infusion. Your doctor will prescribe a corticosteroid medication, antihistamine and antipyretic medicine to help reduce the risk of this type of reaction.
Pregnancy: Darzalex may harm your unborn baby. Pregnant women who are using Darzalex should use effective contraception during treatment and for 3 months after this last dose.
Side effects of Darzalex
Common Darzalex side effects may include:
- Nausea and vomiting
- Diarrhea or constipation
- Fever, chills
- Shortness of breath
- Neurological problem
- Dizzy
- Trouble sleeping, feeling tired
- Swelling in your arms or legs
- Muscle spasms, back pain, joint pain
- Cold symptoms such as stuffy nose, sneezing, cough, sore throat
Call your doctor right away if you have:
- Cough has yellow or green mucus
- Chest pain, wheezing, feeling short of breath
- Neurological problems: numbness, tingling, burning pain
- Low blood cell count: fever, chills, fatigue, mouth sores, skin sores, easy bruising, unusual bleeding, pale skin, cold hands and feet, feeling light-headed or short of breath.
This is not a complete list of side effects and others may occur.
Other drug interactions with Darzalex
You may be given medication to reduce your risk of shingles.
Before each Darzalex infusion, you will be given medicines that help reduce the chance of a reaction related to the infusion, including:
- Medications for an allergic reaction (antihistamines)
- Anti-inflammatory drugs (corticosteroids)
- Fever medications (such as paracetamol)
After each Darzalex infusion, you will be given a medication such as corticosteroids to lower the chance of reaction related to the infusion.
If you have a breathing problem such as asthma or chronic obstructive pulmonary disease (COPD), you will be given inhaled medicine that will help with your breathing problem:
- Medicines to keep the airways in your lungs open (bronchodilators)
- Medications to reduce swelling and irritation in your lungs (corticosteroids)
